The story is set in the early 1920s during the Republic of China, where natural disasters, man-made calamities, and wars are unending, causing immense suffering among the people. Yang Yi, a young doctor with hidden dragon spirit power within him, gets caught up in the turbulent power struggles among various factions in Shanghai due to a plague incident.
