A skilled surgeon, Yasuo Shirakawa (Kippei Shiina), attends to 21-year-old Shotaro Kobayashi (Shono Hayama), who comes for a consultation. Shotaro is diagnosed with anal cancer and is discharged from the hospital after surgery, but a few months later, metastasis is discovered. As the cancer progresses and worsens, he suffers unbearable pain, and the prospect of recovery diminishes. His aunt, Akiko (Maki Sakai), who is by his side, is pushed to her limits and pleads with Shirakawa for euthanasia. Shirakawa refuses and earnestly treats Shotaro, but eventually, there is nothing more he can do. Finally, after much suffering, he carries out euthanasia. However, Shirakawa is subsequently accused by Shotaro’s mother, journalist Yasuyo (Sawa Suzuki), upon learning of her son's death. Amid discussions of whether Shirakawa's actions constitute murder or manslaughter, the establishment of a long-debated euthanasia bill begins to seem more feasible. In the background, Yasuyo plots to enact medical reform in Japan while using opposing doctors and the media to block the euthanasia bill. Shirakawa is eventually swept away by the current.
