Ramón (Javier Bardem) suffers an accident during a diving activity and becomes completely paralyzed. Once a handsome and strong man, he must now endure a life of torment. He lives for 26 years in pain that is unimaginable to most and can no longer grasp the meaning of living. He repeatedly applies for euthanasia but is denied. He hires lawyer Julia (Belén Rueda), who gradually comes to understand his perspective through their interactions. She, too, is unfortunately terminally ill, and they grow closer. Julia promises to help Ramón publish a collection of poems, and then they will leave this world together. A young woman learns about Ramón's experience and strongly urges him not to give up on life, and she falls deeply in love with this unfortunate man. However, none of this can resurrect Ramón from the agony of paralysis. Yet, he still waits for Julia to say goodbye to this world together.
