Middle-aged woman Bessie (Diane Keaton) has spent most of her life taking care of her two elderly relatives: her stroke-afflicted father Marvin and her eccentric aunt Ruth. Now, Bessie herself has developed leukemia, and her only hope for survival is if a close relative will provide bone marrow. Reluctantly, Bessie reaches out to her sister Lee (Meryl Streep), with whom she hasn't been in contact for 20 years. Lee is a single mother who initially doesn't want to meet Bessie, but thinking of the troubles caused by her son Hank (Leonardo DiCaprio) setting fires at home, she decides to return to their Florida hometown with him. The sisters start off emotionally well when they first meet, but as time goes on, past grievances begin to resurface. Additionally, Hank, who has been in a mental institution, refuses to undergo the bone marrow test, which hurts Bessie's feelings. Eventually, Hank's relationship with Bessie miraculously improves, restoring the sisterly bond between them...
