In the mid-Muromachi period of Japan, the power and prestige of the ruling shogunate diminished significantly. The ambitious Daigo Kagemitsu (played by Kiichi Nakai), in order to achieve his ambitions, sacrifices his unborn son Hyakkimaru to 48 demons. He makes a pact with these demons, who take away 48 organs, including the eyes, ears, mouth, and nose. Hyakkimaru, born missing organ parts, is thrown into a river but is later saved by a physician and adoptive father (played by Yoshio Harada), who creates prosthetic eyes and limbs for him, naming him Hyakkimaru (played by Sōta Matsukaze), and raises him. In order to break free from his cursed fate and reclaim each part of his body, Hyakkimaru declares war on the 48 demons that possess his organs, embarking on a journey of battles. On his quest to find the demons, Hyakkimaru encounters a self-proclaimed "bandit," Dororo (played by Yūki Shibasaki), who joins him on his journey in pursuit of the sword he possesses.
