Isak (played by Victor Sjöström) has practiced medicine for 50 years and is now nearly eighty years old. He is preparing to return to his alma mater to receive an honorary degree, accompanied by his daughter-in-law. On the way, Isak revisits old places and reminisces about the past. Isak once had a beautiful first love with his cousin Sara (played by Bibi Andersson), but due to his cold and reclusive personality, his brother took advantage of the situation. Now, sitting on the lawn, Isak recalls the past, and the beautiful image of Sara in a white dress comes to mind. His inherently cold and rational character doomed his marriage to fail. Isak's wife could not endure the icy relationship and sought an affair. This domestic atmosphere, coupled with inherited aloofness, led Isak's son to be unwilling to have children and to become estranged from his daughter-in-law. Isak receives the prestigious honorary degree, yet he remains immersed in heavy self-reflection about his past. For this elderly man nearing the end of life, it seems like a journey of spiritual redemption.
