John Kramer (played by Tobin Bell) has been diagnosed with terminal cancer. Just as he is feeling extremely low, a former partner from a cancer support group informs him about a woman named Cecilia Pederson (played by Synnøve Macody Lund) who has developed a new treatment with a high cure rate, saving many patients on the brink of death. Eager to extend his life, John reaches out to Cecilia, only to learn that she has escaped to Mexico to evade the pursuing pharmaceutical companies. Following her careful guidance, John travels to the neighboring country and undergoes the miraculous surgery. However, he soon discovers that the so-called new therapy is a scam, and everyone around him is part of a fraud ring. Angered, John seeks out Amanda and the detective to enact bloody revenge against Cecilia's group...
