The protagonist, Anna (played by Milla Jovovich), accidentally witnesses a murder on the street and is chased by the assailants. In a frantic struggle, Anna is injured and falls into a river, narrowly escaping death. She becomes the only survivor who has seen the killer's face. The police decide to find a breakthrough in solving the serial murder case through Anna. After a week, Anna wakes from a coma only to discover that every face she sees constantly changes and deforms—she can no longer recognize the faces of those around her, including her boyfriend and father. This condition is medically known as "prosopagnosia," or "face blindness." The killer seems to know about Anna's face blindness due to her brain injury. To test whether Anna truly cannot recognize him, the killer repeatedly provokes and tests her through various means. Anna feels the killer is always lurking nearby, threatening her life at any moment. Eventually, the killer confirms that Anna can only distinguish different people by characteristics such as the color of their clothing and tie styles. He decides to take a risk and wears Anna's ex-boyfriend's clothes and tie to meet her, preparing to take her to a secluded place to eliminate her. However, in a crucial mistake, a detail gives Anna the realization that the man in front of her is not her ex-boyfriend, but the very criminal who wants to kill her. After Anna alerts the police, they arrive to rescue her, leading to a life-and-death showdown and ultimately capturing the killer.
