Five assassins, led by Heiji Bunju, pursue Zatoichi along the Koshu Road, attempting to ambush the palanquin he is riding in. However, inside the palanquin is a young woman named Otoyo, holding a baby. Zatoichi had given up his seat to Otoyo, who was suffering from a sudden illness. Upon learning of Otoyo's death, Zatoichi decides to deliver the baby to her husband, Unosuke from Shinshu, in the village of Miyamoto. Carrying the infant, the blind Zatoichi begins his journey with the palanquin bearers, Dohei and Basuke. Yet, the five assassins attack him. After one is slain by Zatoichi's sword, Dohei and Basuke retreat, leaving Zatoichi to continue his journey with the baby. Enraged by the loss of their comrade, the assassins, now joined by their acquaintance Han Gorō, strike at Zatoichi while he is changing the baby's diaper. However, Zatoichi's sword claims the lives of ten men from Han Gorō's gang as well. The blind Zatoichi’s journey, while caring for the baby, is full of hardships. After earning money through gambling one night, he helps a female pickpocket named Oka, who is being chased by a rural samurai, and employs her as a babysitter. Oka, who falls for Zatoichi's boldness, takes good care of the baby despite their frequent bickering. One day, when Oka causes the baby to urinate from the second floor, it splashes onto a passing sumo wrestler named Koteushi, leading to a brawl with Zatoichi. At that moment, four gang members attack Zatoichi, who is without his cane sword, but thanks to Oka’s quick thinking, they manage to escape. As love begins to blossom between Zatoichi and Oka, he decides to visit Unosuke, who has now become a yakuza boss. Unosuke, however, orders his subordinates to drive away Zatoichi and the babe. Infuriated by Unosuke's attitude, Zatoichi entrusts the infant with the monk of the temple, ZuiKo-in, along with a lock of Otoyo's hair. Just then, assassins allied with Unosuke, led by Heiji Bunju, attack Zatoichi with flaming bamboo spears. This is Heiji's plan to counteract Zatoichi's drawing technique. In the midst of fire and attack, Zatoichi defeats the four assassins and their lackeys but spares only Unosuke’s life for the sake of the sweet baby. Tears glisten in the eyes of Zatoichi and Oka as they part ways with the infant.
