In the midst of Japan's Muromachi period, the authority of the ruling shogunate was rapidly diminishing. The ambitious Daigo Kagemitsu (played by Kiichi Nakai) sacrifices his unborn son, Hyakkimaru, to 48 demons in order to achieve his ambitions. He makes a pact with these demons, who take away 48 organs, including the eyes, ears, mouth, and nose.
Upon birth, Hyakkimaru is lacking these organs and is thrown into a river, where he is later rescued by a doctor who becomes his adoptive father (played by Yoshio Harada). The father creates prosthetic eyes and limbs for him and names him Hyakkimaru (played by Sōta Matsumoto), raising him to adulthood. To break free from his cursed fate and reclaim every part of his body, Hyakkimaru declares war on the 48 demons who possess his organs and embarks on a journey away from home.
During his quest to find the demons, Hyakkimaru encounters a self-proclaimed "bandit" named Dororo (played by Yūko Shibasaki), who joins him on his journey in hopes of acquiring the sword that Hyakkimaru possesses.
