Single father Terry Bernard (played by Josh Lucas) is raising his lovely daughter Angela (Beatrice Miller) alone, and he is in love with his daughter's doctor, Elizabeth (Lena Headey). Everything seems to be heading in a positive direction, but relentless hallucinations fill him with anxiety. Previously, Terry underwent a heart transplant, after which the hallucinations began to plague him. He learns that the heart donor is named Jean Viard, a 55-year-old man who died from a head injury. Through the hallucinations and the records on the donation registry, he finds a man named Kevin (Jamie Harrold). From their conversations, Terry vaguely senses that there might be a dark secret surrounding the donation process.
