In a secluded village in southwest China, surrounded by mountains and known for its long-lived inhabitants and unique customs, was granted the name "Changshou Town" by the emperor during the Qing dynasty. One day, an infectious disease suddenly breaks out, and the local officials send a doctor (played by Donnie Yen) to treat the patients. Outside the town, he discovers the ailing Niu Jieshi (played by Huang Bo). After entering the village, the usually mild-mannered villagers behave uncharacteristically, refusing to help Niu Jieshi, treating him as if he were a plague, and even directing their anger towards the doctor for meddling. The old town leader (played by Ma Jingwu) personally leads the town doctor (played by Su Youpeng), a midwife (played by Liang Jing), a painter, and other elders in an attempt to obstruct the doctor's efforts to save Niu Jieshi. Ultimately, Niu Jieshi cannot be saved, and the doctor is unable to determine the cause of the man's sudden death. The villagers' unwelcoming and uncooperative attitude leaves the doctor disheartened. Just as he is about to leave, he accidentally uncovers a secret hidden by the entire town on a cliff. Driven by curiosity, the doctor decides to investigate Changshou Town in secret, and only after encountering a boy does the truth behind the mysteries of the town begin to unfold... This film is adapted from the novella "Child's Play Murder" by Chen Tiejun.
