After studying medicine abroad, Tang Shan (played by Donnie Yen) returns to China and opens a Western medicine clinic in the slum of Zhabei, Shanghai, with his assistant Liu Guobang. The local bully Yu Lao Qi (played by Yu Rongguang) frequently leads people to beat villagers to seize a piece of land. Tang Shan and Liu Guobang intervene to stop the violence and provide free treatment to the injured villagers. To put a stop to Yu Lao Qi, Tang Shan visits the landowner—his former teacher Simon from England—thinking he can save the villagers' land by offering something in return for Simon's help. Yu Lao Qi's sister, Yu Xiao Qian (played by Zhu Yin), has suffered from a throat ailment for years and cannot speak. Believing in fox spirits and thinking they can cure his sister, Yu Lao Qi rejects Western medical treatment. By chance, Xiao Qian meets Tang Shan, whose sincerity moves her, leading her to accept surgery. Meanwhile, children from the slums start to go missing, and Tang Shan discovers that the Axe Gang and Simon are behind it, leading to an inevitable fierce battle.
