A group of five assassins, including the likes of Heiji Bunjin, Shimakuro, Inokuro, Kikukuro, and Tsuruza, ambush Zatoichi as he travels along the Koshu Road. However, inside his carriage is a young woman named Otoyo, cradling a baby. Zatoichi had given up his carriage to the ailing Otoyo. Upon learning of Otoyo's death, Zatoichi decides to deliver the baby to her husband, Unosuke from Shinshu Miyamoto Village. With the baby in his arms, the blind Zatoichi begins his journey accompanied by the carriage pullers, Dohei and Basuke. Yet, the five assassins attack him. The group suffers one casualty from Zatoichi's iai cut. Dohei and Basuke retreat, allowing Zatoichi to continue his journey with the baby. Enraged by the death of their comrade, the assassins enlist the help of Hango, a familiar face, and attack Zatoichi while he is changing the baby's diaper. However, Zatoichi's sword claims the lives of ten members of Hango's family. As Zatoichi juggles caring for the baby, his yakuza journey is filled with hardships. After earning some money from gambling one night, he saves a woman named Okō, who is being pursued by rural samurai and hires her as a caregiver. Captivated by Zatoichi's spirit, Okō takes good care of the baby, even as she often quarrels with him. One day, Okō accidentally causes a ruckus when she lets the baby pee from the second floor, hitting a passing sumo wrestler named Koteushi, sparking a brawl between her and Zatoichi. Just then, a group of four attacks Zatoichi, who is unarmed, but with Okō's quick thinking, they manage to escape. As the bond of love grows between Zatoichi and Okō, he visits Unosuke, who is now a yakuza boss. However, Unosuke orders his subordinates to chase Zatoichi away, who is still holding the baby. Angered by Unosuke’s attitude, Zatoichi entrusts the baby along with Otoyo's hair to the temple monk at the Bodai Temple, Zuiko-in. At that moment, assassins led by Heiji Bunjin, who are allied with Unosuke, attack while setting fire to their bamboo spears, attempting to corner Zatoichi. This was Heiji Bunjin's strategy to counter Zatoichi's iai moves. Amid the fire and danger, Zatoichi defeats the group of four along with Unosuke’s subordinates but spares the life of Unosuke, the baby’s father. Tears shine in the eyes of Zatoichi and Okō as they part with the baby.
