In 1987, on the eve of the Soviet Union's collapse, Romania, still a socialist country, had clear laws prohibiting abortion. Gabita, a female student abandoned by her boyfriend in Bucharest, unexpectedly becomes pregnant. Her roommate and friend, Otilia (Anamaria Marinca), decides to help her have an abortion in secret. Through a friend's introduction, they connect with a doctor willing to perform the surgery and manage to raise a considerable amount of money. However, the money they gathered does not meet the doctor's demands. In the location of the surgery—a cheap motel—the doctor suggests a trade using Otilia's body. To thoroughly help Gabita, Otilia agrees to the doctor's proposal. Afterwards, she begins to reflect on her relationship with her boyfriend and poses a hypothesis: how would he react if she unexpectedly became pregnant? The film's title, "Four Months, Three Weeks and Two Days," refers to the total duration from Gabita's pregnancy to the abortion.
