Stage actress Elizabeth (played by Liv Ullmann) forgets her lines during a performance and subsequently refuses to speak for several months, leading to her being sent to a mental institution. The nurse caring for her, Alma (played by Bibi Andersson), is blamed by the doctor for her lack of diligence, and the hospital arranges for her to take Elizabeth to a seaside villa for treatment. Once at the villa, Elizabeth shows some improvement but still refuses to speak or respond. Alma often talks to her in an attempt to break the silence but receives no response, ultimately confessing her emotional issues with her fiancé. Alma secretly reads a letter Elizabeth wrote to the doctor and discovers that Elizabeth is discussing herself in the letter, leading her to angrily reprimand Elizabeth. Faced with Elizabeth’s utter silence, Alma, in a fit of rage, nearly kills her. Startled, Elizabeth screams "No!" which makes Alma believe she has cured her. However, Elizabeth again refuses to make any sound, compelling her to make Alma her stand-in, having her sleep with her husband and care for her child. Throughout this process with Elizabeth, Alma gradually loses her own identity, silently transforming into Elizabeth's other half.
