The story takes place in a remote town in Pennsylvania, USA, where Autumn (played by Sidney Flanigan) is an ordinary girl living there. Introverted, Autumn has a very distant relationship with her family and lacks friends she can talk to. One day, Autumn is shocked to discover she is pregnant. She goes to the town's hospital for help, but in this conservative community, abortion is not permitted. In despair, Autumn has to figure out how to terminate the pregnancy on her own, but several attempts end in failure. Her cousin Skyler (played by Talia Ryder) is Autumn's only friend and is sensitive and observant; she notices that Autumn has been acting very unusually. After much questioning, Autumn finally reveals her secret to Skyler, who promptly decides to take Autumn to New York for an abortion.
