Casey (Anya Taylor-Joy) is an awkward girl who seems out of place among her peers. One day, while she is preparing to leave with her classmates Claire (Haley Lu Richardson) and Marcia (Jessica Sula), they are suddenly attacked and kidnapped by a mysterious man. The three girls are imprisoned in a dark, cold basement, facing an unknown perilous fate. The kidnapper, a man named Kevin (James McAvoy), is actually a patient with dissociative identity disorder, possessing as many as 23 different personalities. Dr. Fletcher (Betty Buckley) is Kevin's psychiatrist. She discovers that the personalities within Kevin are in conflict, and worse, a 24th personality is slowly emerging—one that is strong, evil, and cruel. If this personality takes control of Kevin’s body, not only will the three girls suffer, but Kevin himself will be in grave danger.
