Wang Shang'en (played by Andy On) from the International Security Bureau leads his subordinates, including Wan Fei (played by Jay Chou), to Jordan to escort scientist Kenner, who is responsible for creating a variant of the smallpox virus. Unexpectedly, Wang Shang'en betrays them and kidnaps Kenner, leaving Wan Fei with a severe head injury, while his love interest from the team dies during the mission. Three months later, the virus developed by Wang Shang'en and his group causes the deaths of everyone on a cargo ship, as he prepares to profit by selling the vaccine. Wan Fei is told in the hospital that the bullet in his head cannot be removed, and the nerve damage poses a life-threatening risk two weeks later. His mother reveals that he has a long-lost older brother. To fulfill his mother's wish, Wan Fei travels to Kuala Lumpur to find his brother Wan Yang (played by Nicholas Tse). On the plane, Dr. Jian, who is traveling with Wan Fei, happens to be the virus expert that Wang Shang'en's group intends to use to replace Kenner. Dr. Jian is kidnapped by Wan Yang, and Wan Fei intervenes to save him. Due to their opposing sides, the brothers can only settle their differences through combat...
