A group of medical school interns mess with Kenneth (Andrew-Lee Potts), a peeping hospital janitor, one night while having fun at a bar near the hospital, mocking him as "Mad Dog." Forced to drink excessively, Kenneth has a seizure and falls into a coma, becoming a vegetative state and facing termination of treatment for euthanasia. Feeling guilty, intern Catherine (Arielle Kebbel) violates protocol and injects Kenneth with an experimental mixed drug, hoping to wake him up sooner, but instead, the drug causes Kenneth’s soul to leave his body and possess others, leading to a brutal revenge spree. By the time Catherine realizes something is wrong, it’s too late, as Kenneth’s spirit is already beyond her control. Who can Catherine trust now, whether stranger or friend, even herself?
