After Japan's defeat, Sanada (played by Jo Shimura) is a community doctor in a poor neighborhood who enjoys drinking and specializes in treating tuberculosis. One day, gangster boss Matsunaga (played by Toshiro Mifune) comes to him for treatment due to an injury on his hand, but Sanada discovers signs of tuberculosis. He advises Matsunaga to get an X-ray, but Matsunaga refuses to accept the reality of his illness and ends up angrily shouting at Sanada and arguing with him. However, after Matsunaga returns home, his symptoms worsen, and subsequent X-rays confirm he indeed has tuberculosis. Though he puts on a brave front, he eventually accepts Sanada's treatment.
Meanwhile, Matsunaga's gang leader, Okada (played by Reizaburo Yamamoto), is released from prison and gradually takes over Matsunaga's position in the gang, leaving the terminally ill Matsunaga with no choice but to gamble everything...
